diiopcfg.txt also reports this:

Site Config Loaded from: WEB Internet Site document only

This means that the DIIOP site configuration document either does not exist or that the site is not configured to run on that server. Since the site document exists take a look at the "Domino servers that host this site:" field to make sure the server is allowed to run this site.
